Leave of Absence Form
Please fill out the information below to obtain an excused leave of absence from a concert. If you know ahead of time that you will be missing more than one rehearsal, or the concert date itself (travel, job commitments, medical reasons, and so on) you are asked to use this form. Remember that only one absence is allowed within any concert rehearsal schedule. If more than one absence occurs, you will be asked to not participate in that concert. Keep in mind that you are allowed to return to the Chorus if there is a place open (we are trying to keep a maximum of 15 per section).
